Access violation (ACCVIO) in DECwindows server log

The Question is:

 
I strongly suspect I have a hardware problem, but I can not get any
convincing diagnostics.
 
When my 3000/300XL boots it fails to successfully execute Windows.
DECW$SEVER_0_ERROR LOG Says

I upgraded Firmware a short while back, but system worked fine
on several startups.
 
Is it time for a service call?

The Answer is :

 
  Please acquire a more recent version of OpenVMS (such as V6.2 or V7.1),
  please acquire the current DECwindows ECO kit for the version of DECwindows
  in use -- DECwindows V1.2-3 and V1.2-4 both have ECO kits available, please
  run AUTOGEN with a reboot, and then please then contact the customer support
  center.  Expect to be asked for information on the graphics controller in
  use. (Depending on the particular controller, you may be asked to install
  the appropriate Open3D kit for your controller and your OpenVMS version.)
