System hang on SHUTDOWN?

The Question is:

 
We are having some problems shutting down our AlphaServer 4000. For the last
shutdown, this is what we did :-
- @SYS$SYSTEM:SHUTDOWN with 3 minutes warning. Messages come up (as normal)
about interactive logins being disabled, and the Decnet network being
shutdown. The first shutdown message is sent to users, then nothing - the
shutdown procedure appears to hang.
 After 5 minutes of waiting, abort shutdown with CTRL-Y
- @SYS$SYSTEM:SHUTDOWN with 0 minutes warning. Shutdown procedure gets as
far as "%SHUTDOWN-I-STOPQUEUES, the queues on this node will now be
stopped", then nothing....... hangs again. We notice that some queues are
stuck in "stopping" state, so we stop/q
ueue/reset those queues, but still shutdown hangs. Wait a few minutes then
abort again with CTRL-Y.
- we run our site-specific shutdown, then try SHUTDOWN again with 0 minutes
warning. Again, gets as far as stopping queues and then nothing.........
Abort yet again with CTRL-Y.
- crash the system with SYS$SYSTEM:OPCCRASH
  Any hints on how to go about resolving this would be appreciated. This
machine is not clustered. It isn't shutdown very often, but shutting it down
has become a difficult thing to do.... The problem appears to have started
after upgrading to OpenVMS V7.

The Answer is :

 
  Check for the available list of ECO kits -- if you have access to DSN,
  please pull down the list of ECOs incorporated into the release and the
  list of ECOs that should be applied to the release.  (The specific word
  "incorporated" is likely a good search keyword in the DSN system.)
 
  Please contact the customer support center for further assistance, and
  for a copy of the above list if you do not have it.
