Reverse-LAT-like IP (outbound telnet)?

The Question is:

 
Is it possible to map a local virtual device to
a remote TCPIP address/port.
 
eg:-
 
Local device --- xyz0:
any interaction with that device wants to go to
IP server 123.123.123.123/port=1234 which
might be a modem or other IO device on a remote
machine.
 
Effectively, emulating LAT functionality, but
over IP.
