Problems upgrading to OpenVMS VAX V5.5-2H4?

The Question is:

 
I need to get a system disk to VMS 5.5-2H4 I read in the release notes, that
I need to go to 5.5 first then 5.5-2, and finally 5.5-2H4. When I do a
BACKUP/IMAGE/VERIFY DKA500:VMS055.B/SAVE_SET DKA0:, the backup works fine.
But when I then try to boot >>>
B DKA0, the boot always fails and goes to HALT.

The Answer is :

 
  Information on the specific VAX system and specific disk would be
  of interest.
 
  The OpenVMS Wizard will assume that the system in question requires
  OpenVMS VAX V5.5-2H4.  The Wizard will also assume that the disk is
  supported by OpenVMS VAX V5.5 (or later) on the particular VAX system
  in question.
 
  With this assumptions, you will require a "remastered" kit for OpenVMS
  VAX V5.5-2H4, or you will need to perform the upgrades from V5.5 to
  V5.5-2 to V5.5-2H4 on a VAX system that is supported by OpenVMS VAX
  V5.5 or earlier -- only once the operating system is running OpenVMS
  VAX V5.5-2H4 can a system that requires that release be booted.
