AlphaServer 4100 device configuration? (TLZ09)

The Question is:

 
After upgrading to OpenVMS 7.2, from V6.2-H3, our TLZ09 SCSI tape no longer
shows up in the device list.  At the pre-boot console prompt,  SHO DEV
reveals that the tape is attached to the system.  Once the system boots, SHO
DEV at the DCL prompt does not
list the tape device.
 
Further, use of IO AUTOCONFIGURE does not detect/load the device.
 
Booting from the VMS7.2 OS CDROM exhibits the same problem. If I boot  from
the 6.2 OS CDROM the device is detected and works properly.
 
I have attempted to connect the tape device to an Alphaserver 4100 and to a
Alphastation 500, both exhibit the same behavior in every regard.
 
The 4100 is using a QLogic ISP1020 based controller.
 
We are using all firmware updates associated with the CDROM that contained
AXP4100 V5.4 firmware. (Which we received in June.)

The Answer is :

 
  First, check for duplicate SCSI unit number settings, considering all
  SCSI devices and all SCSI host controller(s) connected to the bus.
  Then check for any errors logged, and for SCSI bus cabling problems,
  SCSI bus termination problems, and similar.
 
  Though you do not mention that multi-host SCSI is configured, please
  note that magtape devices are not permitted on shared (multi-host)
  SCSI buses.
 
  The KZPBA (ISP1020) controller has local firmware, are there any
  errors (eg: "*** ISP1020 firmware is out of date ***"), and what
  firmware version is currently loaded into the KZPBA?
 
  What SCSI mode is the KZPBA operating in (pk*0_soft_term)?
 
  The current firmware version is V5.6.  Consider an upgrade.
 
  The OpenVMS Wizard will assume that file-based configurations are
  not in use here.
 
  Please contact the Compaq Customer Support Center, as more detailed
  information on the particular SCSI configuration is required.
