The Answer is :

You will want to contact your hardware support vendor or HP Services,
as it is apparent that the existing storage or hardware configuration
is either incorrect -- or some of the hardware involved here is faulty.

If you wish to maintain your own hardware, please see the information
in the OpenVMS Frequently Asked Questions (FAQ) around third-party
storage considerations, and particularly around the availability of the
  Assisted Services program.

Disk-relevent limitations within the KA41-A series processor console
software are also specifically called out within the FAQ; please look
for references to the MicroVAX 3100 Model 10 and MicroVAX 3100 model 20
series. (Similar limits exist for the KA41-B used in the VAXserver 3100
model 10 and the VAXserver 3100 model 20.) All of these systems have
  system disk capacity limitations.
